Digital planner
  • Instant download, nothing ships
  • Works with Goodnotes, Notability & any PDF app
  • Yours to keep, no subscription

This family medical binder gives each person in your household a health profile and a medication list, with pages for doctors, visits, logs and health history: 54 undated, hyperlinked pages in calm greige and sage for Goodnotes and iPad, and printable too. It's an instant digital download with nothing shipped, and an organizer only, not medical advice.

You get one PDF: 54 undated pages in 7 tabbed sections.

Works in Goodnotes, Notability and Noteshelf on iPad, and in Xodo or Samsung Notes on Android tablets: any note app that keeps PDF links.

What to record, section by section

Front (3 pages). A cover with lines for your family name and the last-updated date, a contents page, and How to Use, which covers the tabs, privacy (no passwords or portal logins) and opening the binder at appointments so your medication list, allergies and questions are in front of you.

01 Health Profiles (8 pages). Family Health at a Glance lists six people with birthday, blood type, primary doctor and allergies; tap the arrow to open a profile. Each of the six profiles holds date of birth, height and weight, an emergency contact, allergies with reactions, current medications with a link to that person's full list, conditions and devices, notes for caregivers, the care team and a preferred hospital, plus a "last reviewed" line.

02 Doctors & Insurance (8 pages). Emergency contacts, with 911 and Poison Help (1-800-222-1222) printed in and "or your local number" beside each. Two doctors & providers pages with 12 roles, from primary care and pediatrician to physical therapist and lab. Insurance & pharmacy (health, dental and vision plans with member ID last 4, and pharmacies including mail order). Two medical bills & claims logs: billed, insurance paid, what you owe, paid on and where the claim or EOB is kept, with a strip for where HSA or FSA receipts are kept. And Legal & Care Wishes, which records where documents such as an advance directive or health care proxy are kept, not what they say.

03 Medications (10 pages). Six medication lists, one for each profile, with dose, how often, what it's for, prescriber, Rx number and pharmacy, refills, and start and end dates; over-the-counter items and supplements go here too. Then three undated medication trackers: write the month, list each medication and time, and check off each dose across 31 day columns.

04 Appointments (7 pages). Two appointment logs (date, time, person, doctor, reason, follow-up, done), two Questions for My Doctor pages with five questions and answer lines plus changes since the last visit, and two visit notes pages for what was discussed, medication changes and next steps.

05 Logs & Journal (7 pages). Two symptom journals (what you noticed, when, how strong from 1 to 5, how long), two vitals logs with columns for blood pressure, pulse, blood sugar, weight and temperature, and two test results logs that note where each full report is kept.

06 Health History (6 pages). Two immunization records (vaccine, dose number, date, where, lot number, next due), allergies & conditions for the whole family, surgeries & hospital visits, and a family health history marked by mother's or father's side.

07 Caregiver & Notes (5 pages). Caregiver info (daily routine, help needed with meals, medication reminders, moving around, dressing and bathing, what comforts the person), caregiver daily notes with one row per visit or shift, and two notes pages.

An organizer, not medical advice

Every page is a blank for your own records. Nothing in the binder suggests a dose, a schedule or a normal reading: the vitals log has no ranges printed, and it reminds you to ask your healthcare provider what your readings mean for you. Follow your healthcare provider's guidance, and keep the official copies of your records.

The Calm Neutral design

Greige and stone pages, soft-white cards, charcoal text and muted sage tables, with a line icon beside every page title and binder-ring holes down the left edge. The cover sets the title beside a warm taupe panel with a heart in a circle and four icons around it: a stethoscope, a pill, a pulse line and a calendar.

Who it's for

  • Parents keeping several children's records, immunizations and appointments straight.
  • Adult children helping a parent, with caregiver pages for anyone who shares the care.
  • Anyone who sees several doctors and wants one list of medications to bring along.

Prefer florals? The same 54 pages come in Warm Floral. Pair it with the Family Emergency Binder for contacts, plans and document locations, and browse both in Family & home.

Questions

Can I print it? Yes. Choose landscape and "fit to page" on US Letter or A4; the light pages print cleanly. To bring one page to an appointment, export just that page from your app and print it.

How many people does it cover? Six profiles and six matching medication lists. For more, duplicate a page in your app; you'll find the copy in your app's page view.

Does it tell me what my readings or doses should be? No. It's a place to write down what you're told and what you measure. Ask your healthcare provider about your own numbers.

Can a caregiver use it? Yes. The caregiver info page and daily notes are made for a handoff between family members or helpers.

Can I share it with my doctor's portal? No. It's a PDF you write in, so nothing connects to a portal or app; keep portal logins out of it.
